Health Alliance PCN is looking for an experienced Advanced Nurse Practitioner (ANP) or Advanced Clinical Practitioner (ACP) to join our Extended Access Hub, based at Belmont Health Centre in Harrow.

The service runs outside core GP hours and is mostly a minor illness clinic, with 15-minute appointments. You'll assess, diagnose, treat and manage a wide range of undifferentiated presentations autonomously, within your professional scope of practice, as part of the wider PCN team.

We welcome applications from suitably qualified ANPs and ACPs with the appropriate registration, advanced practice qualifications and primary care, urgent care or out-of-hours experience.

• Review and manage patients with long-term conditions, as clinically appropriate within the Extended Access service.

• Work autonomously within your professional scope of practice, using sound clinical reasoning to manage risk and make safe decisions.

• Prescribe as an independent prescriber, in line with local formularies, national guidance and safe prescribing standards.

• Request and interpret investigations where clinically appropriate, and act on the results.

• Safety-net appropriately, and escalate or refer on to the right urgent, emergency or onward care when needed.

• Keep accurate, timely and contemporaneous clinical records on the practice clinical system (EMIS).

• Follow PCN and practice policies, including safeguarding, infection control, information governance and clinical governance.

• Work as part of the wider PCN team, communicating well with colleagues across the Extended Access Hub and member practices.
