The Medical Scribe accompanies clinicians during patient encounters to capture relevant information including presenting complaints, clinical history, physical examination findings, investigations and management plans. The scribe must translate verbal clinician notes into coherent, structured entries in the patient record while maintaining confidentiality and clinical accuracy.

The role requires close collaboration with physicians, nurses and administrative staff to clarify documentation needs, track pending results and ensure follow up actions are recorded. Scribes work under supervision and are expected to manage competing priorities, maintain attention to detail during busy clinics and adapt to varying clinical specialities and workflows.

Performance is measured by accuracy of documentation, timeliness of record completion, adherence to clinical documentation standards and the ability to support clinicians in delivering efficient patient care.

Medical Scribe: Duties and Responsibilities

  • Document patient histories, review of systems and current medications during clinical encounters.
  • Record physical examination findings, procedure notes and real time clinician observations.
  • Enter diagnostic test orders, results and interpretations into the patient record as instructed by the clinician.
  • Compose clear and concise assessment and plan entries consistent with clinician direction.
  • Ensure documentation is completed promptly after each patient encounter to maintain up to date records.
  • Maintain strict patient confidentiality and handle sensitive clinical information appropriately.
  • Coordinate with administrative and clinical staff to track follow up appointments, referrals and outstanding investigations.
  • Flag missing or unclear information to the clinician and summarise clarifications in the record.
  • Support coding and billing teams by providing accurate clinical context for encounters when requested.
  • Manage workload across multiple clinicians or examination rooms where required by the service.
  • Adhere to departmental documentation standards, clinical policies and data governance requirements.
  • Participate in training, feedback sessions and quality improvement activities related to documentation practice.
  • Produce legible, standardised entries that facilitate continuity of care between shifts and services.
  • Maintain professional conduct when interacting with patients and clinicians and represent the clinical team appropriately.

Medical Scribe: Requirements and Qualifications

  • High school diploma or equivalent; postsecondary qualification in health sciences or related field preferred.
  • Demonstrable knowledge of medical terminology, anatomy and common clinical conditions.
  • Previous experience in a clinical or healthcare environment is desirable.
  • Proven ability to type accurately and efficiently with good keyboard skills; minimum 50 words per minute preferred.
